Wasn't Andrew Jackson shot right next to the heart, but he still shot a guy to death while the bullet was in.

Either way you either hit a bone or puncture a lung, depending on how deep the bullet could travel.
He put his hand over it and got his shot in, and then another afterward.
Not everyone feels that kind of mid-duel adrenaline and would likely fall over. Or he could've just been that tough.

like somebody else said, your arm has a major artery going through it, and even if you somehow miss that you'd still probably forget up a vein, and end up leading them to die if not treated correctly
