You don't "decide", you accrue enough karma or something in order to truly see the "bad guys" (whatever their names were) weren't bad guys (like you find out in Metro 2034), which unlocks the "good ending".

I used to build my own custom games and invite random people to play them that I played matchmaking with. Met some good friends doing that. Too shy (or just don't want to) nowadays to even consider that.

Those were really fun times.
