All of these include allergic reactions, which is addressed by "rare medical conditions are exempt"
Most of the rest include phrases such as"It is not clear whether these mild or serious problems were caused by the vaccine or occurred after vaccination by chance.", "These are so rare it is hard to tell if they are caused by the vaccine.", or "Because these problems occur so rarely, we can’t be sure whether they are caused by the vaccine or not."
For those that don't use a phrase like that, the meaning is implicitly there because these type of things are heavily biased towards reporting
higher risks; I've been told by someone in the pharmaceutical industry that clinical trials must report all deaths, even if the death is completely unrelated to the medication being tested
