I don't think I enabled any Add-Ons other than my usual. Are there any Add-Ons o Add-On combinations that are known to disallow Self Delete, and also perhaps mess with damage.

what changed from when it was working to now, when it isn't?
if you didn't add/remove addons from when it was working, did you change any configs?

Try updating Slayer
There once was a bug that disabled Self Delete for some reason.

you can also run a trace to definitively find out what is breaking the command

You need to get the Bleach addon. Drink it and you will die.

Run server.
Put trace(1); into the console.
Try suiciding.
Put trace(0); into the console.
Close server.
Post console log.
