Yeah, those would be cult leaders, they tend to get disposed of pretty quickly by their underlings who recognize the threat of having their power structure annihilated. Thankfully all of the countries with nuclear weapons are inclined not to use them, because even though North Korea may rattle on about how they'll destroy the U.S., they're smart enough to not cause any real trouble.
The United States and Russia have systems in place that work well enough for the people in charge right now, what do you think will happen when these structures are threatened by potential nuclear annihilation? And at the very least, the major nuclear powers are relatively stable, you should recognize that nobody wants things to get worse. A war with Russia means diminished U.S. influence across the globe and the potential collapse of the systems that brought people like Hillary Clinton and Putin to power- by this same token, these systems keep them rich and powerful. Yeah sure, it might generate some profits for arms companies in the short term, but that doesn't matter when you can't spend that money due to the global financial system collapsing or the developed nations being annihilated.
