Noble Six is killed by Energy Sword as the camera swings up during that final cutscene (a trope known as "Gory Discretion Shot"; keeps it tasteful but implies what is happening). After expending all his ammo, losing his weapons and taking several hits (his shields are down and not recovering), Six is worn down and literally surrounded by Elites who close in on him, pin him to the ground and end his suffering.
A little depressing but it makes sense. Would have been cool if he just kept going until Reach itself fell, but that wouldn't have been very realistic.

Because there was no avenue of escape, and Six is a proud warrior.

The Pillar of Autumn was the last of the cruisers to escape Reach before the glassing. Anything smaller than a Cruiser wouldn't have the fuel or engines necessary to get to another system (there were no safe planets within Reach's vicinity), and would struggle to slip the net the Covenant had created with their ships surrounding Reach. Anybody left behind was left for dead at that point.

Six was born on Reach, the SPARTANs as a concept were born and raised on Reach, the hope of humanity was sheltered on Reach and his entire team (which he probably considered family at that point) died on Reach. It makes sense he would take one last opportunity to go down fighting for the military's homeworld, even though it was a futile effort.

If you're wondering why he wasn't on the Autumn; after spending a while retrieving the Cortana AI from Halsey and getting to the PoA (which was already carrying John-117 and was intended as the last hope for humanity on Earth), a whole bunch of Covenant ships arrive to forget the PoA's chances of getting off world. Six stayed behind so he could jump on a MAC Cannon and buy the PoA a window to escape.

In the lead-up to the extermination of the Jedi Order, Ahsoka Tano was framed for significant crimes, removed from the Jedi Order and got into some nasty stuff to clear her name (including hanging out with Asajj Ventress). The whole ordeal (which also affected one of her closest friends) meant that while she was offered to the chance to rejoin twice, she declined both times and eventually proceeded on to other things.

She was nowhere near any of the Jedi operations or the Clone Forces during the so called "Jedi Purge", and was totally forgotten about.

I'm rather cynical regarding new Star Wars media, so I'm only cautiously interested at this point. I don't very much like the villains at all.

AC2 and ACB got some much needed loving to fix a lot of bugs and make the games look loving gorgeous (be aware that Brotherhood is incredibly frustrating to play thanks to its stuffty combat system, however). Revelations is still a load of arse. Has been nice to go through my favourite era of the canon, though.
