They will randomly spawn in new chunks.  I have only found 3.

Because too many people are trying to update at the same time.  I updated as soon as Notch said on twitter that the update was out, took me around 2 seconds.

for some reason you have to have at least three portals for it to work you can delete the first one you made and use the one it spawned but you can't delete the one it spawned or it will make another one
