I was messing around in Half life source when i saw this..

It looks like alyx.

No, Valve always does this kind of thing in the games. In episode 2 they had some sort of tunnel that shown the words "How did you get up here?" in the chase section of the antlion thing.

Eli is the scientist who sends you to the surface to get help because all the phones are out. You can only presume when he lost his leg, but it's likely he lost it during his escape from Black Mesa, is it must've been fraught with danger.
