That's normal Misery.
Ugly-ass Misery looks like she came straight out of Betelgeuse.

You don't even have to leave nor examine it. I just talked to him again (knowing the badge isn't the right item) and killed the easy boss.

On final cave hidden. Question, does this lead directly to the Doctor? What I mean by that is there any enemies after the final cave? I don't want to waste time and health getting all those triangle containers. :3
