Depending on the game for difficulty, really.
Some games I can get really good at over time,
others don't really have a "difficulty", such as sandbox games and MMORPGs.

1) I prefer 3rd person games. First person games tend to have limited content and limited views. That and I don't find enjoyment dying 5 seconds into playing.
2) Future or medieval are typically the genres I am drawn to. I love Futuristic because I'm always interested in where Humanity is headed. Medieval because it was a historically active and interesting time.
3) RPG/RTS are usually my games of choice. They usually take the longest to play so I feel that I get a lot more for the money I pay.
4) I like my games challenging, but not to the point where it is downright difficult to play. I like the difficulty to vary so that it isn't overwhelming and I also love to be able to have the option to change it if it isn't challenging enough/too challenging. Difficulty is best when it is based on a player perspective rather than having set limits. It keeps games from becoming far too easy.

7. Chanlenging, but  not impossible and not too easy.

For a platformer with a walljump, an Eight is nice.
Anything else I like it easy but hard when it needs to be.

if its a generic shooter: 8
generic platformer: 7
generic RPG: 5

I'm personally a very easy-quitting player. In Cave Story, there was this boss. I tried to beat him twice then quit the game.
He was like, the first boss. :P But I didn't want to play that entire segment over again so I quit.
I used to play everything on easy.
But now, I like to play medium and hard.

I tend to play pretty casually, so I'll often go on the lowest difficulty. Some games (like Just Cause 2 for instance) I'll use a trainer for infinite ammo/health cause I'm just looking for fun, not a challenge. There's something about being an unstoppable tank I like in that game. I enjoy difficult games sometimes though; like for Mass Effect 2, which is a blast on insanity.

Keyboard, for most games.
For third person games though, I think a controller is best suited for one.

keyboard and mouse over everything
christ i suck at using controllers and they are unfun

Depends on the type of game.

Depends on the game, but for the most part I prefer a controller.
I only hope that Nintendo/PS3/Microsoft quit with the motion controls with their next consoles (though I doubt they will), I prefer normal button mashing.
