In Fallout 3 you could facial surgery. I don't know if you can in F:NV.

Idk about not giving the player a choice, because some people are undoubtedly prejudiced towards ghouls and would rather shoot themselves then become one. I think it'd be better to make it a perk.

Also, I wish the Cyborg perk actually granted a cosmetic change.
