There's yet another Stargate mod for HL2, entitled " 'The' Stargate Mod "

I'd say it looks like the best if it didn't have the stuffty modeled unstable vortex. Needs moar particle one.

SourceForts is alright, but it's a bit laggy if you have a bad comp.
