IIRC, you can use it to make a fake dead body and run away invisible.

It is an item in Team Fortress 2, one of the three cloaking devices available to the Spy class. Instead of manually cloaking and uncloaking, it allows a player to fake death - being shot with the item out will pretend to instantly kill them (showing a death message and awarding 'kill' points temporarily) but actually cause them to turn invisible and allow them to run away for a few seconds, hopefully sneaking up on the person who's now satisfied that the Spy is dead.

Besides, a watch can just be some cubes put to together with a pipe looping through and a timepiece than that can be used as a timer as to how long the invisibility will last.
Or something.
