I found this Half Life 1 HEV plate at Eli's lab, but I forgot to get a picture of it. Believe me, it's there.
But I found another in white forest in episode 2. I also forgot to get a picture of this one, but there is a video of it:

in the chapter waterhazzerd right after you fight the chopper take the pipe with the grate that has a hole in it ( use your airboat ) and you will find a vortigant.
