No, sometimes it's China, some unidentified middle-eastern country (or terrorists from an unidentified middle-eastern country), and somewhat less common, North Korea.
You will never see US fighting the UK or modern Germany or France or anything like that. Come to think of it, I can't name a single FPS that features the modern militaries of France, Germany, Canada, or pretty much any country that isn't the US, UK, Russia, or China.
