   A second Merchant is acquired when there are at least 3 players present who have more than 50  combined.
   A second Nurse appears when there are 3 or more players with a combined health of at least 400. [Might have changed as of 1.0.6]
   A third Merchant spawns when there are at least 5 players with a combined total wealth of more than 5 .

Pfft, I've had 3-4 of the NPCs spawn within 10 seconds of eachother. Volga is my witness.
