So, what do you think the story will be about?
Clearly his wife and kid are gone, but where did they go? Maybe you go for a search to find them Fallout 3 style, or are you tracking down their killer? What are your theories?

You dun got froze in the start, or you got cloned maybe. You leave the vault and catch wind that some dudes matching your family's description/from vault 111 have been spotted and you go find them. along the way the BoS get involved and something about the enclave in mentioned. You discover your family is still alive somehow and then idk what happens

Female protagonist confirmed, so unless the neo-50's America was somehow more progressive than today's America I don't see how a missing wife could be the main plot point.

I bet it's like John Wick where the dog starts the whole thing.

Idea: both characters you make in the character set up get used in the game. your wife survives by some means (survived same way you did most likely) but got out first and it now out in the wastes and you have to find her for some reason (maybe she has your child who maybe also survived?)

Dude most definitely wakes up from some sort of cryogenic sleep.

Possibly the experiment that vault had was to make people go into that sleep and then release people early and leave one person in cyro an extra 100 years.

But why was he the only survivor? Did his family die? Did they leave?

He killed the entire vault and now is either running away from his problems/going on a murderous rampage

The REAL protagonist is the dog and the human is possible companion and love interest

Since it's an elevator door I'd imagine that the whole group made it in.

I really want the protag to be an android with the memories of the man or woman implanted into it.

You jews forgot that the protagonist was kept alive for 200 years, there would be no baby nor spouse, even if they did live
