Make a new file called
Difficulty_hard.cfg
with notepad, and copy-pasta in the code below.
Put this file into the
[ steamlocation ]\Steam\steamapps\common\left 4 dead\left4dead\cfg\
folder, then when starting a campaign or versus on a local server, say
exec difficulty_hard
into the console. You must be the host. It is suggested you do this after the map loads(don't have to do for each map though)

Notes:
+ Makes zombies more alert, a lot less of the time will you be shooting at unalerted or puking zombies.
+ Makes stealth and caution more important
+ More intense-- mobs have a higher chance to show up, and groups of normal zombies can quickly become a mob if you aren't careful.
+ Can be used on any difficulty. Advanced + Difficulty_hard is something like between advanced and expert, so if you are too good for advanced, but not good enough for expert, try this on advanced.
+ Zombies have slightly more health. This is hardly noticeable as most of the weapons are overkill.
+ Zombies have better vision and hearing, and are overall more alert
+ Turns sv_cheats on. You may be able to turn it off (sv_cheats 0) without the mod being effected.(not tested, just realized that now)
This is not a gamebreaker. I just hosted a pub server and we beat Dead Air on advanced with this on. It was intense. In some of the long hallways, where usually you'd have a few seconds or two, then some got alerted, and then it increased from there, a few random shots sent the entire hallway into chaos, and we were surrounded. For those of you that like to snipe puking, unalerted zombies, have no fear. As long as you keep your distance you are fine. Their vision and hearing are improved, not maxed.
It adds a different flavor to the game, but it still feels natural and the flow is good, but it ought to help some of you who have played every campaign 12 times over get some more fun out of the game.
